This website is operated and owned by RISE Physiotherapy Ltd Bristol. Throughout the site the terms “we”, “us” and “our” refers to RISE Physiotherapy Ltd Bristol. We offer this website, including all information, tools and services available from this site to you, the user, conditioned upon your acceptance of all terms, conditions, policies and notices stated here.
By visiting our site and/or purchasing/using our services, you engage in our “service” and agree to be bound by the following terms and conditions, including those additional terms and policies referenced herein and/or available by hyperlink. Theses terms and conditions apply to all users of the site, including without limitation users who are browsers, venders, customers, me and/or contributors to the content.
Please read these Terms and Conditions carefully before accessing or using our website. By accessing or using any part of the site, you agree to be bound by these Terms and Conditions. If you do not agree to all the terms and conditions of this agreement, then you may not access the website or use any services. General Conditions
We reserve the right to refuse service to anyone for any reason at any time.
You understand that your content (not including credit card information), may be transferred unencrypted and involve (a) transmissions over various networks; and (b) changes to conform and adapt to technical requirements of connecting networks or devices. Credit card information is always encrypted during transfer over networks.
You agree not to reproduce, duplicate, copy, sell, resell or exploit any portion of the Service, use of the Service, or access to the Service or any contact on the website through which the service is provided, without express written permission by us.
The headings used in this agreement are included for convenience only and will not limit or otherwise affect these Terms.
Terms of use of RISE Physiotherapy services
When purchasing products and services through you agree to the following terms:
Consent:
- You agree to undergo a physical assessment and treatment. You understand that while physiotherapy is generally safe, it may involve manual therapy or exercises that cause temporary soreness.
- Right to Withdraw: You have the right to stop or refuse any part of the treatment at any time.
- Accurate History: You agree to provide a full and accurate medical history. RISE Physiotherapy is not liable for issues arising from undisclosed medical conditions.
- In line with our privacy policy we will use Rehabguru and Heidi to compile and store notes, by agreeing to the T&C’s you are consenting to this use.
Appointments & Cancellations. We value your time and ours. To keep the clinic running smoothly:
- Cancellation Policy: We require at least 48 hours’ notice for any cancellations or rescheduling.
- Fees: Cancellations made with less than 48 hours’ notice, or “no-shows,” will be charged at 100% of the consultation fee.
- Late Arrivals: If you arrive late, your session will still end at the scheduled time to ensure the next patient is seen promptly. The full session fee will apply.
- We reserve the right to turn anyone away if deemed not appropriate, unacceptable behaviour, unsafe, if deemed unwell or infections against infection prevention policies.
- We reverse the right to ask you to wear a mask/we will PPE during sessions
- To ensure the highest quality of care we limit assessments to one problem per session (at the discretion of the therapist)
- While we try to complete assessment, diagnosis, and initial treatment plan in the first assessment, complex issues may require repeat assessment or second opinion as deemed necessary by the therapist
- You have the right to request a different therapist and we will aim to accommodate where possible
- You have the right to request a chaperone for your assessment, this will be an individual who has completed chaperone training. As a small team covering multiple sites this may limit when appointments are possible and might delay your initial assessment.
Accessibility
- Whilst we do our utmost to be as accessible as possible, some of our sites do require stair access
- It is the responsibility of the client to check access requirement outlined on the booking information prior to the appointment
Fees & Payments
- Payment Timing: Payment is due in full prior to the time of your appointment unless otherwise agreed. We accept card payments, we do not accept cash payments.
- Insurance: If you are using private health insurance, you must provide your authorization code and membership number prior to treatment.
- Patient Responsibility: Note, we do not work with all insurers, it is your responsibility to check this with the clinic prior to booking. If your insurance provider refuses to pay for any reason (e.g., policy excess or exhausted limits), you remain personally liable for the full cost of the treatment.
Data Protection & Privacy
- Records: We are legally required to keep clinical notes for 8 years, or age 25 if treated as a child. These are stored securely and remain confidential.
- Sharing Information: We may share progress reports with your GP or Consultant to ensure continuity of care, unless you explicitly ask us not to.
- Marketing: We will never sell your data. We will only send you clinic updates or exercises if you have opted-in to our mailing list.
- You have the right to request access to your notes via our Clinical Director Simon Carley-Smith
- Occasionally we will use data to send marketing information occasionally, such as special offers, discount codes, referral schemes, clinic updates including opening times, new locations, logistical information
Professional Conduct
- Zero Tolerance: We maintain a safe environment. Any verbal or physical abuse toward our staff will result in immediate discharge from the clinic without a refund.
- We have a zero tolerance policy to abusive behaviour
- We have a zero tolerance policy to clients under the influence of alcohol or drugs
Minors: Patients under the age of [18] must be accompanied by a parent or legal guardian during all sessions.
Accuracy, Completeness And Timeliness Of Information
We are not responsible if information made available on this site is not accurate, complete or current. The material on this site is provided for general information only and should not be relied upon or used as the sole basis for making decisions without consulting primary, more accurate, more complete or more timely sources of information. Any reliance on the material on this site is at your own risk.
This site may contain certain historical information. Historical information, necessarily, is not current and is provided for your reference only.
